Therapeutic Result of Cholecystectomy Performed Through Minilaparotomy and Laparoscopy as Management of Acute Cholecystitis and GB Empyema



Abstract
The advantages of the laparoscopic technique include less pain, shorter hospitalization, and shorter interval to return to full activities, after operation. Laparoscopic cholecystectomy in the past has been advocated for the treatment of
uncomplicated
symptomatic gallstone disease: Acute cholecystitis and GB empyema have been considered to be a relatively or absolute contraindication to laparoscopic surgery. We have treated 102 patients with acute cholecystitis and GB empyema. Laparoscopic
chlecystectomy(LC) was performed in 29 patients(28.4%), Minilaparotomy cholecystectomy(MC) in 25 patients(24.5%), Traditional open cholecystectomy (OC) in 48 patients(47.1%). The conversion from laparoscopic cholecystectomy to traditional open
cholecystectomy was necessary in 4 patients. The conversion from minilaparotomy cholecystectomy was in 3 patients. When compared with traditional open laparotomy, laparoscopy cholecystectomy was associated with shorter mean postoperative hospital
stay,
slightly reduced PFT, less postoperative pain, and cosmetic advantage. The only disadvantage of laparoscopic holecystectomy is that operating time is longer, cost is more expensive than tha for minilaparotomy cholecystectomy or for traditional
cholecystectomy. The minilaparotomy cholecystectomy is more economic and cosmetic advantage then traditional open cholecystectomy. The patient who underwent emergent lapaloscopic cholecystectomy for acute chlecystitis appeared to enjoy many of
the
same
advantages of this procedure as those undergoing elective surgery. The advantage of laparoscopic cholecystectomy was the short hospital stay, early return to normal activity and was small reduction of pulmonary function after laparoscopic
cholecystectomy than after traditional cholecystectomy. We belive that the adequate procedure to patient are important to faster good patient outcomes.
